Demented battle scarred Cat

My cat is 20 years old, has been blind in one eye since being run over as a kitten which also resulted in a wired jaw and artificial hips. He is now basically deaf and very limited vision in his one "good" eye. He lets out a loud series of demented/stressed? yowls on a very regular basis which does not cease during our sleeping hours. Is it time to send him peacefully on his way?
